Cedars-Sinai Medical Center, Center for Digestive Diseases, IBD has an opening for a Nurse Navigator.
The IBD Center - Nurse Navigator is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services – from initial referral to follow-up care.
The NCPC collaborates with the interdisciplinary team. Facilitates multidisciplinary conferences when indicated by complexity of patient’s clinical condition/medical management or other barriers to care. In conjunction with the LIP, provide necessary education and information on available options, the informed decision making process, and realistic goal setting in order to empower the patient/caregiver to actively participate in their plan of care. Establish and coordinate a supportive care plan with goals and interventions based upon treatment and individual patient/caregiver needs, preferences, beliefs and values. Keep patient/caregiver involved in plan and moving toward self-directed care. Review, evaluate and revise the plan of care for appropriateness on an ongoing and timely basis. Be an advocate for the patient/caregiver. Participates in the development of guidelines, protocols, pathways, and other performance improvement activities as it relates to patient care. Participates in the referral of potential research patients to the clinical and translational research teams Serves as a patient advocate throughout the patient care process. Represents the Medical Center at regional and national meetings/ conferences.
The Nurse Navigator adheres to the Nursing Standard of Care/ Practice, the Plan for Provision of Nursing Care, the Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan. He/she maintains competency skills including the ability to provide care to special patient populations and patients with diverse cultural backgrounds. He/she can identify the ability of the patients to follow directions/ instructions and give consent. He/she can identify safety concerns and institute measures to provide a safe environment; and provide appropriate assessment, intervention and evaluation of patients with consideration of developmental milestones in the patient population.

Qualification Requirements/Preferences:
Education Certifications/Licensure Experience Physical Abilities Educational Requirements:
Bachelor's of Science in Nursing degree required.
License/Certification/Registration Requirements:
A current California Registered Nurse License , American Heath Association BLS required.
Care Coordination and Transition Management (CCTM) Certification required within (2) years of hire.
Experience:
2+ years ambulatory care nursing required. Previous experience in gastroenterology sub-specialty and chronically ill long term patients strongly preferred. Strong critical thinking skills essential for coordination of care for patients with Crohns disease, Colitis and IBD symptoms.

• Full sensory response (vision, speech, hearing, smell, touch)
• Manual finger dexterity and fine motor skills with eye-hand coordination
• Frequent standing, walking, twisting (trunk, neck), bending (waist), flexing/extending/stretching (extremities and trunk)
• Occasional sitting, gripping/grasping, reaching below shoulder level, reaching above shoulder level, kneeling, squatting
• Must be able to lift up to 50 pounds, push/pull at least 100 pounds and carry up to 25 pounds
