Job Description

Qualifications: Bachelor’s Degree in a relevant field.  Knowledge of medical terminology, plus two (2) years clinical experience, preferably in an outpatient setting. Two (2) years’ experience as a Licensed Practical Nurse (LPN) or other licensed health professional in the State of New Jersey and/or involvement in community programs preferred. Computer literacy with competence in Microsoft Office components required. Equivalent education, experience and/or training may be substituted for the degree requirements. Must possess excellent written and verbal communication skills; strong organizational skills; detail oriented; able to function independently and work flexible hours; friendly, outgoing, sociable; open-minded & non-judgmental; caring, empathic, compassionate; committed, dedicated, persistent; creative & resourceful. Knowledge of Newark community preferred. Bilingual skills (English/Spanish) preferred.

Responsibilities: The Patient Navigator serves as a chief contact for new patient appointments to the Liver Disease Center of Excellence. The Navigator is a patient advocate and will guide assigned patients through the health care system; ensuring appropriate appointments are given in a timely manner through guidelines agreed upon with the Medical Directors (Medical and Surgical). The Navigator will assist with access issues, develop relationships with referring health care providers and ensure the complete and efficient management of all referrals, including the acquisition of necessary patient-care materials and medical records. This individual will work with community groups to increase health awareness and provide health education. In addition, the Navigator will provide monthly statistics and referring patterns.
