This position will cover the Emergency Department

Our RN Care Coordinators recognize the trust our patients place in them and serve as a vital contact point.These dedicated nurses advocate for their patients and work collaboratively with interdisciplinary teams, keeping the individual needs and wishes of patients and families at the forefront.By becoming an information resource, RN Care Coordinators demonstrate clinical guidance expertise, mobilize community resources, educate patients and families in continuing care options, and function as liaisons with health care insurers to support patients in returning home as quickly and safely as possible.Excellent communication and interpersonal skills will promote our organizational values of respect, integrity, teamwork and excellence.

Bachelor’s degree in Nursing with 3 years related experience required.
License to practice as an RN in the Commonwealth of Pennsylvania.
Experience in case management,care coordination, or discharge planning.
ED-Emergency Department RN experience.
