Florida Hospital Orlando seeks to hire RN Care Manager who will embrace our mission to extend the healing ministry of Christ.

Located on a lush tropical campus, our flagship hospital, 1,107-bed Florida Hospital Orlando, serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. Florida Hospital Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. Department Profile:

The Care Management Department is located in every campus throughout the Florida Hospital system. Our objective is to create a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for services to meet an individuals and family’s comprehensive needs. Care Management is an integrated department with physicians, clinical/nursing teams, ancillary departments and the revenue cycle teams. Care management meets the needs of our patients, families and significant others throughout the care continuum including the post-discharge needs.

Work Hours/Shifts:

Full Time: Mon-Fri, 2:00 pm to 10:30 pm

Job Summary:

Under general supervision, the System Support Care Manager works with the patient/family, physicians, interdisciplinary team, and external agencies. Utilizes expertise in patient/family assessment to assist in creating and/or finalizing a discharge plan. Is responsible for collaborating with the interdisciplinary care team to facilitate the discharge plan as needed and serves as an internal resource to the nursing supervisor to work through problematic/crisis situations. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Skill in utilizing Microsoft Word and Outlook tools and proficiency in the daily use of standard office equipment such as computer, phone, fax, pager, copier, etc.
Master’s degree in Social Work (MSW)
Three years of acute care management experience
Inpatient or ED Social Worker knowledge (preferred)
Three years of Florida Hospital acute care management experience (preferred)

Licensure, Certification, or Registration Required:

Current/active Licensed Clinical Social Worker (LCSW) (preferred)
Case Management certification – ACM (Accredited Case Manager), CCM (Certified Case Manager) (Preferred)

Job Responsibilities:

Physically located at Orlando campus and available by phone during assigned hours for outside campuses. Covers IP discharge planning activities for all seven campuses – includes but not limited to: psycho-social assessments; end-of-life, hospice, and palliative care; family conferences; community care transitions; and physician resource for plan of care for difficult patients.
Participates in crisis intervention in the ED, substance abuse acute intervention, self-neglect alert intervention, and/or DCF (Department of Children and Family Services) intervention.
Ensures compliance with federal, state, and local hospital and accreditation requirements.
Collaborates with the interdisciplinary team, as appropriate, to help facilitate discharge. Applies knowledge of community resources to the development and/or finalization of an appropriate discharge plan.
Establishes and maintains extensive knowledge of social resources and other community programs.
Provides available information for patients to make an informed choice regarding resources/ providers. Advocates for the patient in obtaining and negotiating resources to meet patient needs.
Promotes the patient’s self-determination in all decisions and assists the healthcare team with understanding and respecting the choice(s) of the patient or legally authorized person (LAP).
Interviews patient/LAP for post-discharge planning and/or confirmation of discharge plans, when appropriate.
Utilizes professional judgment along with information obtained from information systems and other healthcare professionals to develop and facilitate the discharge plan for patients. These post-care needs may include, but are not limited to, any or all of the following: obtaining prescriptions, arranging home health care, obtaining durable medical equipment, placement in an ALF, SNF or other housing arrangement.
Appropriately prioritizes cases according to assessed needs, per department standards. Communicates with Care Management team on cases needing next-day follow-up. Escalates issues to Care Management Manager/Director of the campus, as needed.
