Critical Illness Recovery Hospital 

Case Management Coordinator

At Select Specialty/Regency Hospitals, a division of Select Medical, our specialty hospitals care for chronically and critically ill or post-ICU patients who require extended hospital care. We are currently seeking a Case Management Coordinator to join our team. We offer an excellent network of career opportunities across the U.S.

Responsibilities

The Case Management Coordinator assures there is ongoing communication between the patient, their family and the clinical team within a positive cohesive team environment. This position will also provide clerical and department assistance.

Welcomes newly admitted patients and their families to the hospital by reviewing what to expect the first few days, encouraging open flow of communication, setting Care Conference, educating on Patient Satisfaction Survey process. Follows up on missing Advanced Directive, POA or Guardianship documents for copy placement on the medical record.
Assumes responsibility for the completion of the following with the support of the Case Management staff: Daily Projected Discharge Calendar, Weekly Commercial Net Revenue Report, Monthly Charge Audit, UR Quarterly Report, QAPI, Medicare Average Length of Stay Calculator
Under the direction of the Case Manager, coordinates team processes for the intention of keeping track of the patient's clinical timeline including yet not limited to Interdisciplinary Team Conferences, the Functional Skills Safety Assessment, Discharge Conference to Home, Patient and Family education, and help to finalize details of the DC Plan.
Conducts follow-up contact and, where appropriate, documentation of DC Planning Referral outcomes, Non-Medicare UR reviews.
Researches difficult-to-find providers of medical services necessary for safe transitions of care.
Initiates and properly files medical record documentation such as the completed IDT forms, Discharge Conference Worksheets and Case Management Progress Notes.
Schedules patient follow-up appointments and documents on discharge/transfer records.
Initiates DC Planning referrals including yet not limited to SNF, HH, DME and OP Hemodialysis. Refers all clinical questions or issues to the Case Manager for resolution.
Transcribes Med-Host data entry including completed UR by CM.
Creates and forwards medical record packets for medical information exchange of Non-Medicare UR and DC Planning Referrals under the direction of the Case Manager using the most up to date electronic method available.
All other duties as assigned by supervisor.

Qualifications

Bachelors degree preferred, Health Care and Customer Service experience a must.
Goal oriented with a sense of urgency and persistence.
Must have the ability to prioritize and organize multiple cases at any given time.
