The Patient Navigator is a PCMH (Patient Centered Medical Home) team member with clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the clinical process, a navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the patient’s care continuum.
•Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices.
•Initiates communication with patient upon receiving a referral of a patient
•Provides support and guidance throughout a patient’s care continuum
•Assist the patient with navigating through our network of physicians, clinics, hospitals, outpatient centers, patient-support organizations, and other components of our health care system
•Conducts intake for target group to determine their psycho-social, housing, and/or resources needed to obtain maximum benefits from all available sources and makes recommendations
•Assists the medical team in educating the patient and family on resources available concerning treatment and diagnosis and answering any questions related to diagnosis
•Maintains open communication with providers, caregivers and patients in order to coordinate services within, with external facilities, and within the community
•Assists in increasing access to culturally appropriate, supportive care when and where possible
•Links patients, caregivers, and families with appropriate follow-up and support services
•Is able to be available to the patients and families throughout all phases of the care continuum, including prevention, diagnosis, treatment, and transition
•Provides recommendation for individual counseling, family education, advocacy, and case management
•Ensures that all needs of patients are addressed and fulfill PCMH guidelines in a timely manner
•Assist patients with obtaining external resources
•Ability to work independently or as part of a team to accomplish patient goals
•Documents all patient encounters in the clinic EMR
•Other Duties as assigned


• Associates degree required; Bachelors degree preferred
•Minimum of (3) three years working in a hospital, long term care facility or clinic setting with complicated patients
•BLS certification within 6 months of hire (as applicable to department)
•Willingness to be crossed trained to other areas
•Knowledge of PCMH preferred, willingness to learn PCMH required within the first three months of hire
•Organizational skills are required
•Good written and verbal communication skills required
•Ability to read, write and speak English
•Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians
•Bilingual in English and Spanish, fluent in Spanish
